So, you are in La Romana and considering this trip. Brave. Here is the thing ...
First, you'll be picked up around 7 AM by a tour guide that might call a trip to Samana, not Los Haitises, that's where you are going. Check the name on his list, took us 5 min to realize he was there for us.
Second, they say you get a light breakfast, you don't. Make sure you get something at the resort, even a small to go bag. You'll travel for about 40-50 min in a nice AC van, dropped off on a motorway don't get scared, the actual tour bus will pick you up from there, joining a larger group and you will drive for another 2 and a half hours to Samana. Will be no breakfast, just some water offered.
Then a boat ride to the caves, nice scenery, a bit of info about ... well, I'll leave that part for the guide. Boat ride again then to a private sort of island, where you will have a buffet lunch, take your usual selfies, maybe even swim hard to get changed there thought as facilities are not the best ... or nonexistent.
Ride back, nice scenery, more pictures opportunities bring your GoPro or equivalent, is a lot of splashing, and then back to bus/van and, 12 hours later, you are back to the resort.
Would I recommend this tour? Tricky, is a long day, tiring and all you are left with are some nice pictures I hope Viator will allow me to post them after this review, I don't have them processed just yet and the feeling that you have not learned something huge about DR ancestors. However, if you are in Dominican Republic, in an all inclusive resort as I was and you need a break, give it a try.

Too many different languages required for the guide to converse with all the customers. Shallow knowledge by guides on what was being seen. Very basic amenities on the Hondo part of the trip with no obvious changing facilities if you wanted to swim assuming you had remembered to take a towel, the site itself being in need of a general clean up. Basically this is a boat ride with some Taino history thrown in! Maybe I'm too demanding but I cannot recommend this trip to anyone unless you are happy simply with a boat ride and looking inside a cave briefly!
